0 Quick Apply 1 day ago Full Job Description The Case Manager is responsible for planning and coordinating activities related to assisting patients in meeting their coordination of care needs, including providing basic consultation with the oversight of the treatment team, coordinating with family or social supports, coordinating with community providers, and discharge planning.
Developing and implementing a plan of care designed to meet the patient's post hospitalization needs while ensuring continuity of care.
No supervisory function.
Education:
Bachelor's Degree from an accredited college or university, in Social Work or a clinical related mental health field preferred.
Experience:
Minimum of one (1) year experience in related disciplines with hospital experience strongly preferred.
License:
N/A.
Additional Requirements:
Crisis prevention/management training within 30 days of employment and prior to assisting in a restraining procedure.
May be required to work flexible hours.

The check can be for multiple reasons such as signing bonus, supplies, etc. The victim will be instructed to deposit the check and use the money for any of these reasons and then instructed to send the remaining funds to the fraudster. The check will bounce and the victim is left responsible.
